Crown Products ENGLISH Placing a stainless steel crown when a custom adaptation is required 1. Select the proper size crown from the kit by choosing one which seems to have a mesio-distal dimension equal to that of the original tooth. See #1 above. 2. Confirm by comparison with a preoperative measurement. 3. T  rial fit the crown. If there is excessive blanching of the gingiva with the crown in proper occlusion, some trimming is indicated. 4. Establish length of the crown. The easiest method to assure proper trimming of a crown is to scribe a line on the buccal and lingual crown surfaces following the contour of the gingiva. 5. Trim the crown. Using crown scissors, trim 1 mm below the scribe line. Be careful to blend cutting junctions to avoid producing tissue irritating burs. Use a heatless stone to eliminate burs if necessary. 6. Contour trimmed areas. Reform portions of the margin which were trimmed using crown contouring pliers. 7. Test fit. 8. Check for proper contact. Use dental floss to determine if contact points are still properly located. Check for opening between crown and preparation with an explorer. 9. Check occlusion. 10. Finish contour for “snap fit”. If contact points must be re-established, use the Unitek contouring pliers to reshape the interproximal walls. 11. Crimp as required. Finish adapting the margin to fit snugly all the way around the crown, using crimping pliers. 12. Thin and polish margins. To prevent tissue irritation in the custom trimmed areas, the crown margins should be thinned and smoothed using a large heatless stone. Then polish with a rubber wheel, bristle brush or other appropriate instrument to make the crown easier for the patient to clean. Trial fit one more time before proceeding with cementation. 13. Follow cementation steps 5 through 11 of previous section. Note: This product is comprised of a stainless steel alloy that contains elemental nickel and chromium. A small percentage of the population is known to be allergic to these metals. If allergic reaction occurs, direct patient to consult physician.  3M™ ESPE™ and Unitek™ Stainless Steel Adult and Pediatric Crowns Stainless steel crowns, are designed to fill the need for long-term provisional coverage on adult molars and bicuspids as well as pediatric posterior and anterior teeth. 3M ESPE Stainless Steel Crowns are prebelled, precrimped, and prefestooned for minimal trimming and contouring when placing the crown to fit. These crowns are work hardened and have a decreasing cervical wall thickness for better marginal adaptation. Unitek Stainless Steel Crowns offer sizes in adult bicuspids and pediatric anteriors in addition to molars for both. The Unitek stainless steel crowns have parallel walls for broad contacts and shallower occlusal anatomy requiring less occlusal reduction. The thick occlusal surface is designed to help prevent bite-through. Placing a stainless steel crown when custom adaptation is not required 1. S  elect proper size crown by measuring mesio-distal width in patient’s mouth with boley gauge. Once the correct millimeter size has been established, measure crowns in kit to verify correct dimension equal to that of the original tooth. 2. Confirm by comparison with a preoperative measurement. 3. Trial fit. Slip the correct size crown onto the preparation and note its occlusal relationship to adjacent and opposing dentition. For ease of seating, apply mandibular crowns over the lingual, then press down across the broad buccal surface. With maxillary crowns, seat the buccal first, then the lingual. Check for any openings between the crown and preparations with an explorer and reform the margin if necessary to close any minor gaps. 4. Recheck Occlusion. Remove the crown and recheck occlusion for differences in the maxillary-to-mandibular relationship. If the crown prevented the opposing dentition from proper interaction, additional reduction of the preparation may be performed and fit verified. If a Unitek crown has been selected, the margin should be crimped all the way around the crown until the crown fits snugly on the preparation. 5. Isolate the tooth. Rinse the area and isolate the tooth with cotton rolls. Wash the preparation and dry thoroughly. 6. Mix cement and fill crown. Mix cement according to manufacturer’s instructions and fill the crown, care should be taken to avoid air entrapment. 7. P  ress crown onto preparation. Utilize the seating procedure discussed in Step 3. Allow excess cement to flow out under pressure. Note: Do not attempt a final seating with heavy thumb pressure. 8. Quickly examine the crown in relationship to surrounding teeth and check occlusion. 9. Have patient bite crown onto occlusion. This will assure a firm, natural seating in ideal occlusion with opposing dentition. If a wooden dowel, tongue blade, or plastic instrument handle is used to improve force distribution, remember to bridge as much of the quadrant as possible mesio-distally. Note: Never place one of these implements only between the crown and its opposing tooth. This could cause the crown to be driven too deeply into the gingival sulcus and below the natural occlusal table. 10. Remove excess cement. Follow cement manufacturer’s instructions as to the proper waiting period and then remove excess cement with a scaler, explorer or similar instrument. Clean the interproximal areas by drawing a piece of knotted floss back and forth under the contact points. Make sure the gingival sulcus is washed free of excess cement. 11. The finished crown should approximate, as nearly as possible, the original tooth in terms of space occupied in the arch, proper relationship with adjacent teeth and functional occlusion with opposing dentition. An X-ray should reveal a solid seating of the crown on the occlusal surface of the preparation and a gap free marginal fit.  Cleaning and Disinfection of 3M ESPE and Unitek Stainless Steel Adult and Pediatric Crowns Cleaning and disinfection are not required for new stainless steel crowns. The following instructions can be used for cleaning and disinfection prior to use, if desired. Caution: -D  o not use metal brushes or steel wool to clean the stainless steel crowns. - Do not use glutaraldehyde for disinfection of the stainless steel crowns. - Do not mix stainless steel crowns of significantly different sizes in the same container during cleaning and disinfection. - The stainless steel crowns must not be exposed to temperatures higher than 136°C. Cleaning Procedures Automatic Cleaning and Thermal Disinfection • Rinse the stainless steel crowns thoroughly under lukewarm running water to remove any visible dust/soil. • Place the crowns into a washer-disinfector basket. • An enzymatic detergent* with a pH of 7.5 - 11 should be used per manufacturer label instructions. • Follow the washer-disinfector instructions for operating procedures.
