Biologisches Gewebe enthält eine mehr oder weniger hohe Konzentration an Elektrolyten. Dadurch ist das Gewebe genügend leitfähig, um elektrochirurgisch behandelt zu werden. Für die Gewebetrennung – Schneiden – und die Gewebekoagulation – Gewebeaustrocknung und/ oder BIutungsstillung – wird die thermische Wirkung des hochfrequenten Stromes ausgenutzt.

Biological tissue contains a more or less high concentration of electrolytes, making it sufciently conductive to be treated electrosurgically. The thermal effect of HF current is used for separating (cutting) and coagulating tissue (desiccation of tissue and/or hemostasis).

Hochfrequente Ströme sind für die Applikation am Patienten notwendig, da niederfrequente Ströme durch elektrochemische Vorgänge (Elektrolyse) im Stromuss liegende Nerven- und Muskelzellen stimulieren können. Der Körper des Patienten ist dabei immer Teil eines geschlossenen Stromkreises. Die Gewebetrennung oder Koagulation erfolgt an der aktiven Elektrode.

With the unipolar operation technique, the electric current ows from the small-face active electrode to a large-face passive or neutral electrode, the patient’s body always forming part of a closed circuit. Tissue separation or coagulation takes place at the active electrode.

Bei der Bipolaren Operationstechnik ießt der Strom zwischen 2 je nach Anwendung (Schneiden oder Koagulation) ächenmäßig gleich oder unterschiedlich großen gepaarten Elektroden. Bei der Anwendung ist nur ein kleiner Teil des Gewebes zwischen den EIektroden in den Stromkreis eingeschlossen.

Um biologisches Gewebe zu koagulieren ist es notwendig, dieses auf ca. 70 °C zu erhitzen. Um eine gewebetrennende Wirkung des Stromes zu vermeiden, muss die Stromzufuhr und damit die Wärmemenge gezielt zur Denaturierung des Gewebes eingesetzt werden.

Aufgrund der Inhomogenität der elektrischen und thermischen Eigenschaften im Gewebe und der daraus ableitbaren ungleichmäßigen Stromdichteverteilung des Gewebes steigt die Temperatur bei Stromzufuhr unregelmäßig verteilt an. In der Nähe der Koagulationselektrode ist die Stromdichte und somit auch die Temperatur am höchsten. Die Temperatur nimmt mit dem Abstand von der Elektrode sehr schnell ab, so dass Koagulationen zunächst auf das Umfeld der Koagulationselektrode beschränkt sind. Die effektive Kontaktäche zwischen Gewebe und Koagulationselektrode hat einen entscheidenden Einuss auf die maximal erreichbare Ausdehnung der Koagulationszone.

Due to the inhomogeneity of the electrical and thermal properties of tissue and the resulting uneven current density distribution within the tissue, the temperature rises with an irregular distribution when power is supplied. The current density, and thus the temperature, is highest in the vicinity of the coagulation electrode. The temperature decreases very rapidly with increasing distance from the electrode so that coagulation is at rst limited to the area surrounding the coagulation electrode. The effective contact area between the tissue and coagulation electrode has a decisive inuence on the maximum achievable area of the coagulation zone.

Wegen der Abhängigkeit der räumlichen Ausdehnung der Koagulationszone von der Stromdichteverteilung ist auch die Applikationstechnik des HF-Stromes von Bedeutung. Bei der SOFT-Koagulation wird die Generatorspannung so gering gehalten, dass es während des Koagulationsvorgangs zu keiner Lichtbogenbildung zwischen Elektrode und Gewebe kommen kann. Die SOFT-Koagulation ist für alle Koagulationen empfehlenswert, bei denen unipolare oder bipolare Koagulationselektroden verwendet werden, die direkt mit dem zu koagulierenden Gewebe in Kontakt gebracht werden. Die SOFT-Koagulation ermöglicht tiefe Koagulationen, wobei eine Karbonisation des Gewebes vermieden wird.

With SOFT coagulation, the generator voltage is kept so low that no arcing is able to take place between the electrode and tissue during the coagulation process. SOFT coagulation is recommended for all instances of coagulation in which unipolar or bipolar coagulation electrodes are used which are brought into direct contact with the tissue that is to be coagulated. SOFT coagulation provides for deep coagulation while avoiding carbonization of the tissue.

Bei der Forcierten Koagulation werden elektrische Lichtbögen zwischen Koagulationselektrode und Gewebe bewusst zugelassen, um oberächlich verschorfte, jedoch in die Tiefe reichende Koagulationen zu ermöglichen. Dies gilt insbesondere dann, wenn für die Koagulation nur kleinächige Elektroden zur Verfügung stehen. Um die Gefahr eines Schneideeffektes zu vermeiden muss eine modulierte HF-Spannung verwendet werden. Eine Verschorfung des Gewebes muss hierbei allerdings in Kauf genommen werden.

With forced coagulation, electric arcs are intentionally permitted between the coagulation electrode and tissue to enable the formation of surface sloughing with nevertheless deep-reaching coagulation. This particularly applies if only small-face electrodes are available for coagulation. To avoid the risk of a cutting effect, a modulated HF voltage must be used. Tissue sloughing will, however, occur.

Bei falscher oder fehlender Applikation der Neutralelektrode besteht ein hohes Risiko einer thermischen Gewebeschädigung sowohl an der Applikationsstelle der Neutralelektrode wie auch an anderen Körperstellen des Patienten. Bei der Applikation von Neutralelektroden ist deshalb unbedingt darauf zu achten, dass die effektive Kontaktäche bzw. der Übergangsleitwert zwischen Neutralelektrode und Patient der verwendeten HF-Leistung bzw. der Intensität des HF-Stromes adäquat ist. Mit effektiver Kontaktäche ist hier die Fläche der Neutralelektrode gemeint, die während der HF-Chirurgie elektrisch leitfähigen Kontakt zur Haut des Patienten hat. Bei der Auswahl der Neutralelektrode sind folgende Erfahrungswerte mit zu berücksichtigen:

Überwachung der Neutralelektrode Das AUTOCON® II 80 verfügt über eine Sicherheitsschaltung, welche die elektrische Verbindung zwischen Gerät und Neutralelektrode überwacht. Bei Verwendung zweiflächiger Elektroden wird zusätzlich die korrekte Applikation am Patienten kontrolliert. Die Statusanzeige Neutralelektrode (NE) zeigt die Qualität der Anbringung der NE am Patienten an. Bei korrekt angebrachter NE wird dies durch eine grün dargestellte Neutralelektrode angezeigt. Wird ein kritischer Grenzwert unter- oder überschritten wird dies rot angezeigt.

Monitoring the neutral electrode The AUTOCON®II80 has a safety circuit which monitors the electrical connection between the neutral electrode and unit and, when using doublefaced electrodes, the correct application on the patient. The neutral electrode (NE) status display indicates how well the NE has been positioned on the patient. If it is positioned correctly on the patient a green neutral electrode is displayed. The neutral electrode monitoring system of the AUTOCON®II80 monitors the electrical connection between the neutral electrode and the unit for single and dual surface neutral electrodes. The skin contact is also monitored for dual surface neutral electrodes. All KARLSTORZ neutral electrodes are correctly monitored. When using third-party neutral electrodes, you must check in the accompanying papers of the manufacturer whether the neutral electrode is suitable for the AUTOCON®II80 used. Partial detachment of a single-faced neutral electrode always increases the current concentration under the neutral electrode and results in thermal damage to tissue due to the limited contact area. If the connection is interrupted or the neutral electrode positioned poorly (NE symbol lights up red) the power output in the unipolar operating modes is blocked.

WARNUNG: Falsche Applikation der Neutralelektrode Verbrennungsgefahr für den Patienten! • Applizieren Sie die Neutralelektrode mit ihrer gesamten Kontaktäche an einer gut durchbluteten, muskulösen Stelle. • Applizieren Sie die Neutralelektrode so nahe wie möglich am Operationsfeld. • Schieben Sie die Kontaktlasche der Neutralelektrode vollständig in die Anschlussklammer. Die Kontaktlasche darf die Haut des Patienten nicht berühren. • Richten Sie die Symmetrielinie der Neutralelektrode in Richtung Operationsfeld aus. Der Strom soll von der aktiven Elektrode (Instrument), auf die Symmetrielinie der Neutralelektrode zuießen (siehe Abb. 1). • Überprüfen Sie die Neutralelektrode regelmäßig auf guten Kontakt zur Haut des Patienten. • Überprüfen Sie die Neutralelektrode insbesondere, wenn der Patient umgelagert wurde und nach Operationsschritten, bei denen das Gerät häug und lange aktiviert wurde. 1. Bringen Sie die Neutralelektrode am Patienten an. 2. Bei Neutralelektroden ohne fest verbundenes Anschlusskabel: Verbinden Sie die Neutralelektrode mit dem Anschlusskabel. 3. Kontrollieren Sie die Neutralelektroden-Kontrollleuchte neben der Neutralelektroden-Buchse (Abb.2). Anzeige grün: Die unipolare Buchse kann aktiviert werden. Anzeige rot: Die unipolare Buchse kann nicht aktiviert werden. Wenn Sie bei roter Kontrollleuchte versuchen, die unipolare Buchse zu aktivieren, ertönt ein Signalton und die Fehlernummer »2« wird angezeigt.
