INTRODUCTION

General Description The MTS Base is a multi-function surgical support that utilizes both electrically operated and hand operated controls. The height adjustment and Trendelenburg adjustment are performed using a pneumatic hand operated switch that controls two electromechanical actuators. Patient rotation is performed by the operator and is controlled by a manually applied friction brake and position lock system. Specifications The MTS Base height adjustment range, at the centerline of the crossbar, is 21 inches to 37 inches. The Trendelenburg and Reverse Trendelenburg range is 10° in each direction achieved by simultaneously raising one end and lowering the opposite end to the desired position. The overall length of the Base is 106 inches and the overall width is 33 inches. The Retractable Base collapses to a storage length of 48 inches. The patient weight limit of the MTS Base is 500 pounds in a prone, supine, or lateral position. NOTE: Although the MTS Base is capable of supporting a patient of 500 pounds, the ultimate patient weight limit is determined by the weight limit of the specific table top selected. Refer to Table 1. for weight limits for the available tops. Table 1. indicates the patient weight limits for each available top. It is the responsibility of the operator to verify that the appropriate table top is used for each patient. The weight limit of each top is clearly indicated on a label on the table top.

BASIC OPERATION

Major Component Identification The major components of the MTS Base are identified in Figure 1.

The general nomenclature and orientation of the table is shown in Figure 1. Refer to the appropriate Table Top Technique Manual supplied with each top for Component Identification and more detailed information about each table top.

Control Operation Plug the Power Cord into the proper receptacle depending on the voltage requirements of the MTS Base. Refer to the label at the Head End Base for input voltage requirements. (See Figure 2.) Turn on the power switch and observe the green light in the switch indicating that power is applied to the table. This rocker switch is a combination power switch, circuit breaker, and pilot light. Turning this switch off and then back on will reset the circuit breaker if tripped. Hand Control Module The table is equipped with a hand-held Hand Control Module which is found in a storage bracket attached to the Head End Column. The Hand Control Module allows the operator to raise, lower, Trendelenburg, and Reverse Trendelenburg the Table. Each column is independently controlled with an up or down button on the Hand Control Module. (See Figure 3.)

To Raise the Table, simultaneously depress the two buttons on the right side of the control with the arrows that point in the up direction. (Head Up and Foot Up buttons). To Lower the Table, simultaneously depress the two buttons on the left side of the control with the arrows that point in the down direction. (Head Down and Foot Down buttons).

To Trendelenburg the Table, simultaneously depress the button on the left with the arrow that indicates Head Down and the button on the right that indicates Foot Up until the desired Trendelenburg angle is achieved. This lowers the Head End Column and raises the Foot End Column simultaneously. If the Table is in a lowered position, it may only be necessary to also raise the Foot End to achieve the proper Trendelenburg angle. In this instance, depress the button with the arrow that indicates Foot Up to raise the patient's feet. To Reverse Trendelenburg the Table, simultaneously depress the button on the right with the arrow that indicates Head Up and the button on the left that indicates Foot Down until the desired Reverse Trendelenburg angle is achieved. This raises the Head End Column and lowers the Foot End Column simultaneously. If the Table is in a raised position, it may only be necessary to lower the Foot End to achieve the proper Reverse Trendelenburg angle. In this instance, depress the button with the arrow that indicates Foot Down to lower the patient's feet.

INSPECTION

Acceptance & Transfer Upon receipt of your MTS Base, remove any protective wrapping or packaging. Visually inspect all surfaces for freight damage. Check each caster for proper lock, unlock, and rolling operation. NOTE:

Any freight damage must be reported to the freight carrier immediately upon delivery. It is the responsibility of the recipient to make freight damage claims.

Verify receipt of the Completeness and Test Report provided with each MTS Base. Read the Model/Serial Number Identification Label found at the Head End Base (See Figure 2.) to confirm the input power requirements. After reading Section II. BASIC OPERATION of this manual, plug the power cord into the appropriate receptacle, and turn on the power switch. Operate each of the four buttons on Hand Control Module, and confirm the operation of the following functions, Head Up, Head Down, Foot Up, and Foot Down. Operate the Patient Safety Transfer lock at both the Head and Foot End of the table. Operate the 25° Rotational Lock at the Head End of the table. Disengage both Head and Foot End Patient Safety Transfer Locks, the 25° Rotation Lock, and both Rotational Friction Control Locks. Now rotate the Crossbar at each end of the table one complete revolution, and re-engage all locks. Report any malfunction to your Mizuho OSI representative immediately. Preoperative Inspect and test the Base as described in Acceptance and Transfer above prior to each surgery. Confirm that all components required for the surgical procedure are accessible and operational prior to the surgery. Postoperative Inspect and test the Base as described in Acceptance and Transfer above after each surgery. Clean the Base as described in Section VIII. CLEANING after each use. Annually Inspect and test the Base as described in Acceptance and Transfer above. Thoroughly clean the table as described in Section VIII. CLEANING. Pay special attention to the cleanliness of the center support bar on a Retractable Base as excessive soil could affect function. Check the operation of the Rotational Friction Control at both the Head and Foot Ends. It should be fully locked when at the proper position as indicated on the label. Adjust if necessary according to the procedure in Section V. ADJUSTMENTS. Inspect the power cord for cuts in the insulation or damage to the connector. Inspect the Hand Control Module and the coiled hose for damage or leaks, proper operation of each function will verify the integrity of the Hand Control components. Check the Foot End Slider Assembly for smooth travel over its full range.

THE ELECTRICAL SYSTEM

Description The electrical system is comprised of a power cord, an electrical circuit breaker, junction box, wire harness, and two electromechanical actuators (columns). Drawing number 5891-4 is the Electrical Wiring Diagram for the Model No. 5890 Fixed Base. Drawing number 5890-12 is the Electrical Wiring Diagram for the Model 5890 Retractable Base. The input voltage requirement for these two models is 120 VAC, 60Hz. Drawing number 5891-4 is the Electrical Wiring Diagram for the Model Numbers 5890I Fixed Base and 5891I Retractable Base 220 VAC, 50Hz. These drawings can be found in the Appendix at the end of this manual. All bases are ETL and CSA listed as well as CE marked. Circuit Breaker An external circuit breaker is positioned at the end of the Head End Base. (See Figure 1. for location). A short circuit will cause the breaker switch to trip and can be reset by simply pushing it to the off position and then turning it on again. NOTE: Older units may be equipped with a circuit breaker installed in the Junction Box under the Head End Base or a fuse inside the electrical Junction Box. The circuit breaker switch also serves as a pilot light indicating whether the power switch is on or off. Power Cord The MTS Base is equipped with a power cord with the appropriate 120 VAC hospital grade connector. The power cord is connected in the Junction Box under the Head End Base near the column. Junction Box The electrical Junction Box is located under the Head End Base and contains the electrical connection point for the input power cord, the power switch/circuit breaker, and the wire harness to the columns. There are two electro-pneumatic switches connected to a circuit board inside the electrical Junction Box. These electro-pneumatic switches convert pneumatic pressure from the Hand Control Module into an electrical signal which operates each column. Wires are connected to the circuit board by shielded screw terminals. The hand control pneumatic coiled hose is connected to the electro-pneumatic switches by small diameter tubes and connection is made by barbed fittings. Columns The Head and Foot End Columns are self-contained electromechanical actuators. There are no serviceable parts inside the columns. The columns are connected to the wire harness by means of a 5-pin connector located at the base of each column. If a column fails internally, the entire column must be replaced. NOTE: The columns are designed to operate continuously for 60 to 80 seconds. If continuously operated longer, a thermal switch will automatically stop the motor allowing it to cool. The thermal switch will reset automatically in 5 minutes, allowing further operation of the column.

Servicing the Electrical System NOTE: Unplug the power cord from the wall before removing the access cover. Access to the Junction Box is achieved by first removing any table top from the crossbars, laying the entire MTS Base on its side, then removing the four screws attaching the access cover under the Head End Base. The Junction Box cover is removed by removing the screws in each corner of the box. To diagnose possible line power problems, carefully plug in the power cord. With the power switch on, power can be measured with a voltmeter at each electrical terminal. The PC board input power to the circuit board should be measured between terminals TB1-N and TB1-L. It should be within 10 volts of the specified voltage requirement When the appropriate button is pressed on the Hand Control the voltage between TB2-A1 and TB2-L1 or TB2-L2 should be within +/- 10 volts of 120 VAC. The same applies to TB3-A1 and TB3-L1 or TB3-L2. Refer to the appropriate electrical wiring diagram in Section IX. MASTER PARTS LISTS and DRAWINGS of this manual for the identification of these terminals. Replace the Junction Box cover in the opposite manner as it was removed. NOTE:

Care should be taken when replacing the Junction Box cover so as not to pinch or dislodge the small pneumatic tubes inside the box.

Leakage Current The MTS Base is designed and tested not to exceed the allowable 1000 microamps of leakage current (single fault condition, EN60601-1). Normal values of leakage current range from 14 microamps to 45 microamps normal polarity and 80 microamps to 98 microamps reverse polarity. NOTE: All values below the 1000 microamp threshold are considered acceptable per EN60601-1 Standards. Refer to the Completeness and Test Report supplied with your table for the factory tested values.

ADJUSTMENTS

From time to time certain items may need adjustment from normal usage. These items are: Rotational Friction Controls As the Friction Control wears, adjustment may be required to achieve complete locking in the maximum friction position of the hand levers. The adjustment for this is as follows: Head End Friction Control (Refer to Figure 5)

Loosen the lock nut (11) under the Head End Control Box using a 9/16" box wrench. Loosen the adjustment set screw (12) by turning it counter-clockwise two turns with a 9/32" hex wrench. Set the Rotational Friction Control Lever to the MAX position as indicated by the decal on the Head End Control Box. Release the Patient Transfer Safety Lock (24). Tighten the adjustment set screw (12) by turning it clockwise. Rotate the Head End Crossbar (3) and continue tightening the set screw until the Crossbar cannot be rotated by hand. Hold the adjustment set screw in this position with the hex wrench, and tighten the lock nut (11). Loosen the Rotation Friction Control Lever then retighten to test its locking ability.

Foot End Frictional Control (Refer to Figure 6)

Remove the Foot End Cover as described in Section VI REMOVAL AND REPLACEMENT OF COMPONENTS. Loosen the lock nut (23) on the Friction Clamp Guide (21). Loosen the adjustment set screw by turning it counter-clockwise, two turns with a 9/32" hex wrench. (See Figure 4 on page 8.) Set the Rotational Frictional Control Lever from below to the locked position. Release the Patient Transfer Safety Lock (11). Tighten the adjustment set screw (22) by turning it clockwise. Rotate the Foot End Crossbar (3) and continue tightening the set screw until the Crossbar cannot be rotated by hand. Hold the adjustment set screw in this position and tighten the lock nut. Loosen the Friction Control Lever then retighten to test its locking ability.

Foot End Slider (Refer to Figure 4)

The Foot End Slider is equipped with a drag shoe to set the friction between the slider and top plate of the column. When properly adjusted, 20 to 25 lbs. force should be required to move the Crossbar. To adjust the slider friction: 1 2 3 4 5

Remove the Foot End Cover as described in Section VI. REMOVAL AND REPLACEMENT OF COMPONENTS. Removed ground wire holding screw, insert a long slotted head screwdriver in ground wire screw hole. Insert a long small slotted head screwdriver into the access hole with the adjustment screw. To increase the friction, turn the screw one-half turn clockwise. To decrease the friction, turn the screw one-half turn counter-clockwise. Test the force required to move the slider with a force gauge.

REMOVAL AND REPLACEMENT OF COMPONENTS

Hand Control Module The Hand Control Module can be removed by gently pulling the 4-lumen hose, thereby disconnecting the barbed fittings at the base of the Hand Control Module. Before pulling, remove the strain relief shrink tube at the base of the Hand Control Module. (See Figure 3). NOTE: The proper orientation between the hose and the Hand Control Module is indicated by the red mark on each side of the control hose junction beneath the strain relief shrink tube. Be certain to maintain the original alignment when connecting the hose. The coiled hose is connected to the Junction Box under the Head End Base using the same type barbed fitting found at the Hand Control Module. After removing the access cover, the hose can be disconnected by gently pulling the hose away from the rubber grommet inside the Junction Box. NOTE: Always mark the orientation of the hose to the Junction Box before removal and be certain to reattach the hose in the same orientation. If only one table function fails, an air leak in that lumen of the hose is probable. To distinguish between a leak in the Hand Control and a leak in the hose, disconnect the hose at the Hand Control Module as described above, rotate it 90°, and temporarily reattach it. Operate the suspected failed function button, if another function is actuated then the Hand Control Module is operational. If the original failed table function has now moved to another button, the hose is defective. At this point, replace the coiled hose. There are no serviceable parts inside the Hand Control Module. If it is determined that the Hand Control Module has failed it must be replaced. If none of the functions are operational, the problem is most likely in the electrical system. When operating properly, actuation of a button on the Hand Control Module should cause an audible click inside the Junction Box. If a click can be heard, the problem is most likely a disconnection of one of the connectors at the base of the columns. Push these connectors in firmly to be certain that they are completely seated. If no click is heard, the problem may be with the input power or the small diameter pneumatic tubes inside the Junction Box. Perform the electrical system service testing as indicated on Page 6 of this manual and visually inspect the pneumatic tubes inside the Junction Box. Casters 1. Remove the metric Allen head bolt and the washer from the outside of the base frame near the caster using a 5 millimeter hex wrench. NOTE: The 5 mm bolts are installed with thread locking compound, it may be necessary to heat the bolt with a hot air gun to facilitate removal. 1 2 3

Remove and replace caster. Align the tapped mounting hole in the new caster with the hole in the base frame. Install the Allen head bolt. Apply a drop of removable thread locking compound to the threads and tighten the bolt.

Head End Cover 1 2 3

Remove the two Allen head screws holding the Traction Pulley Bracket. Remove the four #10-32 UNC screws around the lower edge of the Head End Cover. Remove the Rotational Friction Control lever by unscrewing the lever counter-clockwise.

NOTE: The Patient Transfer Safety Lock pin is spring loaded and may be ejected once the lever is removed. 1 2 3 4

Remove the Patient Transfer Safety Lock Lever by unscrewing the lever using an open-end 3/8" wrench on the flats. Remove the 25 ° Rotation Stop lever by unscrewing the lever using an open-end 3/8" wrench on the flats. Remove Head End Cover by sliding it up and off the Patient Transfer Safety Lock lever. Install in the reverse order.

Foot End Cover NOTE: The Patient Transfer Safety Lock pin is spring loaded and may be ejected once the lever is removed. 1 Remove the Patient Transfer Safety Lock Lever by unscrewing the lever using an open-end 3/8" wrench on the flats. 2 Remove the four #10-32 UNC socket head screws around the lower edge of the Foot End Cover. 3 Remove cover. 4 Install in the reverse order. Crossbar - Head End (Refer to Figure 5)

Remove the Head End Cover as described above. Reinstall the 25° Rotation Stop Lever (23). Remove the two 3/8-16 UNC socket set screws (8) in the Brake Drum (4). Remove the three 1/4-20 UNC socket screws (9) in the Bearing Plate (2) and slide the Bearing Plate off of the Crossbar Shaft (3). Remove the two 1/4-20 UNC socket screws (21) and the Head End Brake Stop (20). Rotate the Friction Clamp (5) up and slide the Brake Drum (4) and Brake Band Assembly (14) off of the Crossbar Shaft (3). Remove the Keyway (13). Remove the Crossbar (3). Install in the reverse order.

Cross Bar-Foot End (Refer to Figure 6)

Remove the Foot End Crossbar Cover as described above. Replace the Patient Transfer Safety Lock Lever (11). Remove the two 3/8-16 UNC set screws from the Brake Drum (4). Loosen the Rotational Friction Control. Remove the three 1/4-20 UNC socket head screws (26) from the Bearing Plate (2) and slide the Bearing Plate off to the Crossbar Shaft (3). 6 Remove the 1/4-20 UNC screws (24) and the Foot End Brake Stop (21). 7 Rotate the Friction Clamp (5) up and slide the Brake Drum (2) and Brake Band Assembly (8) off of the Crossbar Shaft (3). 8 Remove the Keyway (9). 9 Remove the Crossbar (3). 10 Install in the reverse order. Head End Rotational Friction Control Brake Band (Refer to Figure 5)

Remove the three 1/4-20 UNC socket head screws (9) in the Bearing Plate (12) and slide the Bearing Plate off of the Crossbar Shaft (3). Rotate the Friction Clamp (5) up and slide it off of the Brake Drum (14). Remove the Brake Band (14). Install in the reverse order.

Foot End Rotational Friction Control Brake Band (Refer to Figure 6)

Remove the Foot End Cover as described above. Remove the two 1/4-20 UNC socket head screws (24) and the Foot End Brake Stop (21). Remove the three 1/4-20 UNC socket head screws (26) in the Bearing Plate (2) and slide the Bearing Plate off of the Crossbar Shaft (3). Rotate the Friction Clamp (5) up and slide it off of the Brake Drum (4). Remove the Brake Band (8). Install in the reverse order.

Head or Foot Column (Refer to Drawing 5890, in Appendix)

Remove any tabletop and H-Frame. Unplug the electric power cord. Lay the entire base on its side. Unplug the 5-pin connector at the base of the column. Remove the four 5mm socket head screws (16 and 17) at the base of the column. Remove the column by sliding it out of the base. Remove the Head or Foot End Crossbar Assembly as described above. Install in the reverse order.

NOTE:

Before tightening the mounting screws (16 and 17) upon installation, stand the base upright and attach a tabletop between the Head and Foot End Crossbar. With the screws loose, rotate the top slowly several times to align the Head and Foot Ends. Reach under the Head and Foot End Base and tighten the mounting screws before removing the tabletop

Center Beam - Fixed or Retractable (Refer to Drawing Number 5890, Sheets 1, 2 and 3 in Appendix)

Remove any table top and H-Frame. Unplug the electric power cord. Lay the entire base on its side. Unplug the 5-pin connector from the base of the Foot End Column and remove any cable clamps or wire ties from this wire. 5 Remove the access cover (38) by removing the three 8-32 UNC (3). 6 Open the Junction Box (49) and remove the wires for the Foot End Column by disconnecting them at the screw terminals. 7 Remove any cable clamps or wire ties on this wire. 8 Remove the four 1/4-20 UNC socket head screws (18) at each end of the center beam. 9 Remove the center beam. 10 Install in the reverse order. NOTE:

Before tightening the 5 mm mounting screws (16 and 17) upon installation, lift the table onto its wheels and attach a table top between the Head and Foot End Crossbar. With the 5mm mounting screws loose, rotate the top slowly several times to align the Head and Foot Ends. Reach under the Head and Foot End Base and tighten the 5mm mounting screws before removing the table top.

Retractable Base Roller (Refer to Drawing Number 5890, Sheets 1, 2 and 3 in the Appendix)

Remove the Center Beam as described above. Completely loosen Knob (55) Remove the four 10-32 x 3/8 Button Head Socket Screws (39) on the Stop Assembly (19) under the beam. Remove the Stop Assembly (19) Carefully collapse the Foot End Beam Assembly (13) into the Head End Beam Assembly (14) so that it just exits the tube. To replace the inside Rollers (51), remove the Retaining Ring (53) on the Bearing Shaft (54) and then push the Bearing Shaft (54) out of the tube. The Roller (51) can be removed from inside the beam. Extend beam until the inside tube just exits the outside tube. Be careful not to over extend the beam as the cable may be damaged if stretched too far. To replace the outside Roller (51), remove the Retaining Ring (53) on the end of the Bearing Shaft (52) and push the Bearing Shaft (52) out of its housing. The Roller (51) can be removed from inside the beam. Install in the reverse order.

Plug in.

Check circuit breaker (green light should be on).

Reset broker, turn on.

Check Hand Control Module and coiled hose.

Plug in hose.

Check electrical connector at base of Head or Foot End Column.

Plug in connector.

Check Hand Control and coil hose.

Plug in hose.

Check electrical connector at base of Head End Column.

Plug in connector.

Listen for air/electrical switch operation in Junction box.

Replace hose or Junction Box.

Check Hand Control Module and coil hose.

Plug in hose.

Check electrical connector at base of Foot End Column.

Plug in connector.

Listen for air/electrical switch operation in Junction Box.

Replace hose or Junction Box.

Rotation Friction Control won’t stop rotation Head or Foot End

Tighten lever to Max. On Foot End check the handle range.

Adjust brake tension.

Check brake band.

Adjust handle to proper range. Replace if necessary.

Caster won’t lock/unlock

Check locking for above wheel for damage.

Replace Caster.

Transfer Lock will not insert into Crossbar – Head or Foot End

Check Crossbar Pivot Shaft for straightness.

Replace Crossbar.

Check Transfer Lock for burr or scratch

Relieve burr.

Check spring.

Replace spring.

Check center beam for excessive soil.

Clean center beam.

Check roller for excessive soil or damage.

Replace or clean roller.

Check lock handle.

Loosen kick handle.

H-Frame won’t connect to Crossbar – Head or Foot End.

Check mounting pin for straightness.

Replace pin (or Crossbar).

Check H-Frame for damage.

Replace H-Frame.

Table Top cannot be attached to H-Frame at Foot End.

H-Frame won’t connect to Crossbar – Head or Foot End.

Replace Crossbar.

Check T-Pin for straightness.

CLEANING

Maintenance Routine table care will assure many years of trouble-free service. For your convenience we recommend the following routine table care. Cleaning And Disinfecting Note:

The Head and Foot columns are not liquid tight. Care should be taken to prevent excessive liquid from entering the extensile joint of these columns. Never pour any liquid onto the table or columns. Never subject the MTS Base to an equipment washing machine

1. Table Exterior a. Exterior surface should be regularly wiped clean with a mild detergent solution and wiped dry with a soft lint-free cloth. b. Care should be taken to avoid exposing the table to excessive moisture. Flooding, fogging or steam cleaning is not recommended. c. Blood or other fluids, etc., if allowed to remain on the table for a long period of time, will require special cleaning to remove. A 5% acetic acid solution or white vinegar and water solution is especially good. d. Staining and discoloration of plated or stainless steel surfaces can be corrected by cleaning with a good commercial cleaning compound, such as Stainless Steel Magic or Acme White finish, and then buffing surface by hand. e. To disinfect exterior surface use a quaternary ammonium or similar type disinfectant compound according to manufacturer’s directions for use. Wipe dry with a soft lint-free cloth.

USE OF IODOPHORS WILL CAUSE STAINING. NOTE: Failure to thoroughly dry surface after cleaning and disinfecting may result in rust. 2. Casters a. Casters should be cleaned and disinfected in the same manner as table exterior. 3. Table Pads a. b. c. d.

Remove pads from table top and clean with mild detergent solution. Do not submerge in water. Rinse thoroughly with clean cloth then dry using lint-free cloth. Disinfect surfaces of table tops. Wipe dry.

4. Table Tops a. Follow the instructions for Table Exterior above. Lubrication 1. Table Exterior Following a thorough cleaning and decontamination of the table, silicone lubricant spray may be used on most of the exposed inner column. No other lubrication is necessary. 2. Table Interior All of the internal moving parts of your MTS Base receive factory lubrication which, under normal use, provides lubrication for the life of the equipment. © Miuzho OSI 2009
